JD - Palliative Care Nurse:

Project Context:

This role of Palliative Care Nurse is part of Defeat-NCD India Foundations Palliative Care Programme. A initiative with Goa Medical College (GMC) is foreseen to establish OPD, IPD, day-care, and home-based palliative care services. Key objectives include:

- Improving quality of life for patients with life-threatening illnesses.
- Reducing hospital readmissions through community-based care.
- Strengthening training for healthcare professionals and caregivers.

The project also focuses on raising awareness and enhancing palliative care capacity in primary and secondary healthcare settings and promoting policy-level advocacy to integrate palliative care into Goas health system.

- Care -coordination of patients registered in the organisation
- Provide critical nursing support and education to palliative care patients and their caregivers across settings IP, OP, Home care
- Adhere to the practice of confidentiality regarding patients, families, staff, and the organisation Provide critical nursing support and education to palliative care patients and their caregivers across settings – IP, OP, Home care.

- Work as part of a multi-disciplinary palliative care team to ensure excellent patient care services.
- Identifying patients with Palliative care needs, enrolling new patients, assessment of their needs and prioritizing their care
- Liaise with and assist the doctor in symptom management
- Triaging to the right care setting as per needs
- Identify the psycho-social needs and help triage to multi-Disciplinary team member
- Out-of-hours phone triage
- Assess informal caregivers’ ability, to provide nursing care/education to patients and family
- Maintenance of nursing record, patient registers – IP, OP, home care, death register
- Work as part of a multi-disciplinary team
- Participate actively in continuing education training programs
- Assist in in-house training sessions and participate in outstation training as required




- Stay updated on best practices in palliative care through training and workshop. Mentor and support others nurses and nursing students as needed.

- Attend educational activities of the unit and participate actively in continuing education training programs

- Participate in events held as part of continuity of care
- Bereavement calls/visits/events (Remembrance Day)
- Cancer awareness, preventive oncology

- Perform other duties as required and assigned by management

Job Requirements

Qualifications

- BSc, GNM (General Nurse Midwifery)

Desirable:

- Computer Skills- MS Office (Word, PowerPoint and Excel)

Experience:

- 3-5 years of clinical experience in Palliative care

Skills/Competencies:

- Exemplify ethical behaviour;
- adhere to the practice of respect, confidentiality, beneficence and non-maleficence regarding patients, families, staff, and the organization.
- Exemplify respectful behaviour with all members of the team and foster the dignity of each member in their role.
- Educate patients /family on clinical condition, hands-on care needs, progress of disease, prognosis
- Discuss and provide end-of-life care with empathy and religious/cultural sensitivity
- Be available out-of-hours – for phone follow-up/triage
- Update knowledge and skills of the science continuously
