Job Roles :

· Review provider documentation and assign accurate ICD-10-CM codes according to HCC / E&M; coding guidelines.

· Validate medical record documentation to ensure diagnoses are supported per CMS and payer standards.

· Identify opportunities for provider education on documentation improvement to capture appropriate scores.

· Collaborate with physicians, clinical staff, and coding teams to clarify documentation as needed.

· Ensure coding accuracy to support Medicare Advantage, ACA, and other risk-adjusted health plans.

· Stay up to date with HCC / E&M; model changes, coding guidelines, and compliance requirements.

· Assist with audits, quality reviews, and coding validation initiatives.

· Maintain coding accuracy, productivity benchmarks,



and HIPAA compliance.

· Ensure compliance with HIPAA and other regulatory requirements.

Desired Candidate profile :

Strong analytical and critical-thinking skills.

Effective communication skills for provider queries and education.

Ability to work independently and as part of a coding team.

Solid understanding of compliance regulations and payer-specific risk adjustment guidelines.

Job Requirement:

· Bachelor’s degree in any life science stream.

· With at least 3 months -1 year experience in HCC/ E&M; coding.

· Strong knowledge of ICD-10-CM coding, risk adjustment models (CMS-HCC, HHS-HCC), and documentation requirements.

· Familiarity with EMR/EHR systems and coding software.

· Excellent attention to detail, accuracy, and compliance awareness.
