Role & responsibilities

We are seeking a Transition Project Manager to lead the planning, coordination, and execution of service and operational transitions between teams, functions, or business units. This role is responsible for ensuring the effective transfer of services, processes, knowledge, and operational responsibilities while minimizing business disruption and maintaining continuity.

The Transition Project Manager will work closely with cross-functional stakeholders to develop transition plans, manage timelines, track risks and dependencies, and oversee handover activities through completion. The ideal candidate will bring robust project management, stakeholder engagement, and change coordination capabilities to ensure a seamless and controlled transition process.

Key Responsibilities
Lead and manage end-to-end transition activities, ensuring delivery against agreed timelines and milestones.
Coordinate between outgoing and incoming teams to ensure smooth transfer of services, processes, responsibilities, and knowledge.
Develop and maintain transition plans, trackers, risk logs, and status reports.




Identify and manage risks, issues, dependencies, and escalations throughout the transition lifecycle.
Facilitate governance meetings, stakeholder reviews, and progress updates.
Ensure all required process documentation, SOPs, and operational handover materials are completed.
Support operational readiness, stabilization activities, and post-transition follow-up.
Work collaboratively with business, operations, and support teams to minimize disruption during transition.

Requirements
5+ years of project or transition management experience.
Experience managing cross-functional operational transitions.
Solid stakeholder management and communication skills.
Ability to manage multiple workstreams and timelines.
Robust problem-solving and organizational skills.
Proficiency with project tracking and reporting tools.

Preferred
Experience in shared services, operations, outsourcing, or transformation programs.
PMP, PRINCE2, Agile, or similar certification preferred.
