Conducting assesments of patient s social ,emotional ,and financial needs to develop care plans.
Providing counselling and emotional support to patients and their families to help them cope with illness ,treatment, and recovery.
Connecting patients with community resources such as support groups ,financial assistance programs.
Advocating for patients rights and interests within the health care system ,ensuring they receive appropriate care & services.
Offering education to patients and families about their medical conditions ,treatment options ,and available support services.
Maintaining accurate and confidential records of assessment ,interventions and referrals in accordance with legal and ethical standards.




Collaborating with interdisciplinary teams ,including Physicians ,nurses ,therapists and managers to coordinate comprehensive care of patients.
