Brief Profile PSP Counsellor (ALP) - Field Home Visits 1. Role of a counsellor involves meeting patients • Meeting patient within 24 hours of receiving request to contact • Sharing the essence of the Patient Support Program • Sharing essential information about the disease, broad treatment guidelines and lifestyle modifications • Carefully listening to the concerns and difficulties faced by the patient • Emphasizing the importance of compliance to therapy suggested by the treating physician • Timely follow-up with patients to assess patient progress towards therapeutic outcomes and also to ensure that the rapport and timely communication to the respective doctor.
- Analysis and documenting of reasons for drop-out.
- Taking doctor and patient consent, and proper maintenance of consent forms.
- Supporting queries from patients not enrolled on Saath 7, only if query is related to use of device 2.
