Project Title:
“Integrated Community Health Program for NCD Screening (Hypertension and Diabetes) and Maternal-Newborn Care in Tea Garden and Forest-Fringe Villages of Kalchini Block,
Alipurduar”
Project Overview:
To improve access to preventive healthcare, mass population screening, and primary care for ~21K people (out of total ~38K population) from ~7.6K households across 7 villages of 2 contiguous GPs in Kalchini block of Alipurduar. This includes screening and management of hypertension and diabetes (men and women aged 30 years and above), antenatal and postnatal care, and home-based newborn care and children below five years of age.
Role Summary:
The Project Manager will be based in Kalchini and hold overall responsibility for the day-to-day execution of the Integrated Community Health Program, operating under the strategic guidance and oversight of the Director. This role is the single point of accountability for translating the program design into functioning clinics, camps, and community outreach on the ground.
