JOB SUMMARY - Operations Coordinator

An Operations Coordinator coordinates/supervises/administers skilled nursing care to patients on an intermittent basis in their place of residence in coordination with the Nursing Supervisor. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Head-Operations and Nursing Supervisor.

QUALIFICATIONS
Graduate in any discipline.
Minimum Three (3) year experience in Home Healthcare Operations/ Defence organization
Ability to exercise initiative and independent judgment.

RESPONSIBILITIES




Performs the initial evaluation visit and regularly re-evaluates the patient’s nursing needs.
Initiates the plan of care and necessary revisions in consultations with Nursing supervisor.
Coordinates services in accordance with the care plan.
Recruit, train and monitor Caregivers reporting to her.
Appoint third party vendors for service requirements.
Prepares roasters, clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per organisation policy.
Counsels the patient and family/significant others in meeting nursing and related needs.
Participates in and presents in-service programs.
Supervises and teaches other nursing personnel.
Conducts patient care conferences on patient assigned to his/her care.SUPERVISION
