Community Tribal Health Nurse(ANM / GNM / BSc Nursing)

Location: Remote Tribal & Rural VillagesReporting to: Program Coordinator

Role Purpose : The Community Tribal Health Nurse will serve as a frontline healthcare provider delivering essential health services in remote tribal villages. The nurse will strengthen last-mile access to healthcare through village outreach, high-risk identification, referrals, and fixed day services, while working respectfully within local cultural contexts.

Key Responsibilities
Community Outreach & Field Services Conduct scheduled village visits and outreach camps. Provide primary clinical care during field visits Support implementation of Fixed Day Health Services Maintain household-level health tracking in assigned villages
High-Risk Identification & Referral Identify high-risk pregnancies, malnourished children, elderly with co-morbidities, NCD cases Stabilize and refer patients to higher facilities Coordinate referral transport and follow-up




Maternal & Child Health Support Antenatal and postnatal monitoring Immunization support (as per protocol)Nutrition counselling and growth monitoring
Community Engagement Conduct health awareness sessions in local language Build trust with tribal leaders, ASHAs, Anganwadi workers Support community-led health planning5. Documentation & Reporting

Maintain patient registers and digital/physical records. Submit weekly outreach reports Assist in data collection for program monitoring

Eligibility ANM / GNM / BSc Nursing Valid nursing registration Experience in rural/tribal health preferred Willingness to travel and work in remote terrain

Community trust-building skills Fluency in local language (preferred)
