We are seeking a detail-oriented, experienced clinical professional to join our Home Healthcare Team as a Quality Assurance Executive / Specialist. In this role, you will leverage your hands-on home health nursing background to assist the leadership team, support field clinicians in the US, and uphold our high standards of clinical excellence.

Position Overview

(Note: This role is strictly for licensed clinical personnel and requires working US night shifts from our Ahmedabad office.)

The Clinical QA Specialist is responsible for reviewing clinical documentation, patient care plans, and agency practices to ensure full compliance with US federal, state, and local regulations. You will play a pivotal role in maintaining care quality through rigorous chart audits, OASIS reviews, and Quality Assurance Performance Improvement (QAPI) initiatives.

Key Responsibilities

- Documentation & Chart Audits: Thoroughly review clinical notes, comprehensive assessments, and Plans of Care (Form 485)



to ensure accuracy, completeness, and timely submission.
- OASIS & ICD-10 Review: Scrutinize OASIS assessments for accuracy and consistency to support optimal patient outcomes and appropriate reimbursement under the Patient-Driven Groupings Model (PDGM).
- Regulatory Compliance: Ensure all clinical documentation adheres to US Medicare Conditions of Participation (CoPs), California Title 22, Medi-Cal, and CDPH regulations.
- QAPI Program: Actively participate in the Quality Assurance Performance Improvement committee; identify trends, track quality indicators, and support corrective action plans.
- Staff Education & Feedback: Provide constructive feedback and ongoing documentation guidance to field clinical staff regarding regulatory updates and best practices.
- Survey Readiness: Assist leadership in maintaining continuous survey readiness for US state, federal, and accreditation bodies (e.g., CHAP, The Joint Commission).
- EMR Systems: Utilize Electronic Medical Record (EMR) software (e.g., Homecare Homebase, Kinnser, Axxess) to manage, audit, and audit clinical records efficiently.

Requirements & Qualifications

- Clinical Credential (Strict Requirement):



Active and unencumbered license as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN). Non-clinical applicants will not be considered.
- Clinical Experience: Minimum 1 year of clinical experience in a Home Healthcare setting.
- QA Experience: Prior background in Quality Assurance, clinical auditing, or case management is highly preferred.
- Certifications (Bonus): OASIS Certification (COS-C or HCS-O) and/or Coding Certification (BCHH-C) is a strong plus.
- Regulatory Knowledge: Solid understanding of US home health guidelines, California Title 22, Medicare CoPs, and PDGM models.
- Language & Communication: High fluency in English (written and verbal) with robust analytical skills and acute attention to detail.
- Work Location: In-person at our Ahmedabad, Gujarat office.
- Shift Requirement: Must be flexible and willing to work US Night Shifts.
