Key Responsibilities

- Assist physicians in patient assessment and clinical documentation.

- Review patient medical histories, medication profiles, laboratory reports, and treatment plans.

- Support medication reconciliation and identify potential drug interactions, duplications, allergies, and medication-related problems.

- Provide appropriate medication-related information to physicians and the clinical team.

- Monitor patients for medication-related adverse effects and therapeutic outcomes.

- Assist in preparing and maintaining patient case sheets and clinical records.

- Coordinate with doctors, nurses, pharmacists, laboratories, and other healthcare professionals.

- Participate in ward rounds and clinical discussions as required.

- Follow up on investigations, reports, and physician instructions.

- Educate patients regarding medications, adherence, precautions, and lifestyle-related aspects as directed by the treating physician.

- Ensure accurate and timely clinical documentation.





- Maintain patient confidentiality and comply with hospital policies and SOPs.

- Support quality improvement, clinical audits, and patient-safety initiatives.

Eligibility

- Pharm.D / Pharm.D (PB) from a recognized institution.

- Candidates with internship/clinical posting experience are preferred.

- Freshers are welcome to apply.

- Good understanding of pharmacology, therapeutics, clinical pharmacy, and patient care.

- Good communication and interpersonal skills.

- Willingness to work in a hospital/clinical setting and rotational shifts if required.
