Education

- Pharm.D from a recognized institution.

- Valid registration with the State Pharmacy Council.

Experience

- 5–8 years of relevant experience in clinical pharmacy services, preferably in a NABH-accredited hospital.

- Experience in medication review, medication reconciliation, prescription review and patient counselling preferred.

Clinical Skills & Competencies

- Robust knowledge of pharmacology, therapeutics and medication safety.

- Competency in reviewing prescriptions for appropriate drug, dose, route, frequency and duration.

- Ability to identify potential drug interactions, contraindications and medication-related problems.

- Skilled in medication reconciliation and patient medication history.

- Competent in monitoring and reporting adverse drug reactions.

- Good knowledge of high-alert medications and LASA drugs.

- Effective patient counselling and communication skills.

- Good documentation and clinical record-keeping skills.

Key Responsibilities

- Review prescriptions for appropriate drug selection, dosage, route, frequency and duration.





- Conduct medication reconciliation during admission, transfer and discharge.

- Identify and report drug interactions, contraindications, allergies and medication-related problems.

- Monitor adverse drug reactions and coordinate appropriate reporting.

- Provide medication counselling to patients and caregivers regarding safe and effective medication use.

- Participate in clinical rounds and provide drug-related information to doctors and nursing staff.

- Monitor the use and storage of high-alert, LASA and other critical medications.

- Promote rational and safe use of medicines within the hospital.

- Maintain required clinical pharmacy records, interventions and reports.

- Participate in medication safety initiatives, audits, quality improvement activities and NABH requirements.

- Coordinate with Pharmacy, Nursing and Clinical Departments to ensure safe medication practices.
