Agewell is building a new model of healthcare for older adults that combines clinic-based consultations, home healthcare, rehabilitation, care management, and ongoing support for seniors and their families.

As we launch our dedicated Senior Care Clinic, we are looking for a dynamic, compassionate, and patient-centric physician to join our growing team.

Key Responsibilities

- Conduct consultations and health assessments for seniors at the Agewell Senior Care Clinic.
- Diagnose and manage acute and chronic health conditions.
- Conduct home visits as required.
- Support post-hospitalization recovery and continuity of care.
- Participate in the development and implementation of personalized Annual Care Plans.
- Work closely with care managers, therapists, nurses, and other healthcare professionals to ensure coordinated care.
- Educate and support seniors,



families, and caregivers.

Qualifications
- MBBS from a recognized institution.
- Minimum 3 years of clinical experience.
- Experience in General Practice, Family Medicine, Internal Medicine, Primary Care, Home Healthcare, or Elder Care will be preferred.
- Willingness to travel within the city for home visits.
