- Will be responsible to receive the patients and to ensure that registration is done for current patients.

- MSW will be responsible for collecting demographic details of the patients through the Registration form.

- The MSW will also have to capture details of the referring doctor and patients family doctor details.

- Provide psychosocial assessment and counselling support to patients and families.

- Assist patients in addressing social, emotional, financial, and family-related concerns.

- Coordinate with psychiatrists, psychologists, nurses, and other healthcare professionals.

- Conduct patient and family interviews and maintain appropriate case records.

- Support treatment adherence, rehabilitation, and community reintegration.

- Identify social support systems and connect patients with relevant welfare schemes and community resources.

- Assist with discharge planning and appropriate referrals.

Preferred: Specialization or experience in Medical & Psychiatric Social Work / Mental Health.
