Key Responsibilities:

- Coordinate the preparation and completion of patient discharge summaries.

- Collect and verify required clinical and administrative information from relevant departments.

- Ensure discharge summaries contain accurate patient details, diagnosis, treatment provided, investigations, procedures, medications, and follow-up instructions.

- Coordinate with consultants/doctors for completion and approval of discharge summaries.

- Follow up with concerned departments for pending discharge documentation.

- Ensure discharge summaries are prepared within hospital-defined turnaround time (TAT).

- Check documents for completeness, accuracy, spelling, formatting, and required signatures/approvals.

- Coordinate with the billing department to ensure discharge documentation aligns with the final billing process.





- Handle corrections or modifications as instructed by authorized medical personnel.

- Maintain confidentiality and security of patient information.

- Assist the Medical Records Department with proper filing and maintenance of discharge documents.

- Coordinate with nursing staff regarding discharge instructions and required documentation.

- Escalate delays, missing information, or discrepancies to the appropriate department/authority.

- Follow hospital policies, medical-record procedures, NABH requirements, and applicable confidentiality standards.
