1. Job Purpose

To provide safe, effective and comprehensive anesthesia and pre-operative care for patients undergoing orthopaedic and trauma procedures. The Anaesthetist will be responsible for pre-operative assessment, anaesthesia administration, intra-operative monitoring, post-operative care, pain management and management of anaesthesia-related emergencies.

1. Key ResponsibilitiesA. Pre-operative Assessment

- Conduct comprehensive Pre-Anaesthesia Check-up (PAC) for all scheduled surgical patients.

- Review medical history, medications, allergies, investigations and previous anaesthesia records.

- Assess and optimise patients for surgery, particularly elderly patients and those with co-morbidities.

- Assess airway, cardiovascular and respiratory status and determine anaesthetic risk.

- Develop and document an appropriate anaesthesia plan.

- Explain the proposed anaesthesia and relevant risks to the patient/attendant and obtain informed consent as per hospital policy.

- Coordinate with orthopaedic surgeons and other specialists regarding patient optimisation and surgical planning.

Anaesthesia

Services

- Provide appropriate general, regional, neuraxial and monitored anaesthesia/sedation, according to the procedure and patient's clinical condition.

- Manage anaesthesia for orthopaedic procedures such as:

- Joint replacement surgeries

- Fracture fixation and trauma procedures

- Arthroscopy

- Spine and other orthopaedic procedures, where applicable

- Day-care orthopaedic procedures

- Ensure appropriate airway management, ventilation and haemodynamic monitoring.

- Administer anaesthetic drugs safely and monitor the patient's response throughout surgery.

- Recognise and manage intra-operative complications and emergencies.

C.

Regional

Anaesthesia & Pain Management

- Perform appropriate regional/neuraxial techniques within the doctor's competence and hospital scope of practice.

- Provide peri-operative analgesia and multimodal pain-management strategies.

- Assist in managing acute post-operative pain and other hospital pain-management requirements, where applicable.

D. Intra-operative Patient Safety

- Ensure continuous monitoring of vital parameters during anaesthesia.

- Maintain appropriate anaesthesia records and documentation.

- Ensure availability and proper functioning of anaesthesia equipment, drugs and emergency/resuscitation equipment.





- Follow hospital infection-control, patient-safety and medication-safety protocols.

- Communicate effectively with the orthopaedic surgical team, nursing staff and other healthcare professionals.

E. Post-operative Care

- Assess and manage patients in the recovery/PACU area following anaesthesia.

- Ensure safe recovery from anaesthesia and appropriate transfer to the ward, HDU or ICU.

- Monitor for post-operative complications such as respiratory depression, haemodynamic instability, nausea/vomiting and uncontrolled pain.

- Provide post-operative analgesia and follow-up when required.

F. Emergency & Critical Care Responsibilities

- Attend anaesthesia-related emergencies and provide immediate resuscitation when required.

- Participate in management of peri-operative emergencies, trauma cases and critically ill patients within the hospital's facilities and protocols.

- Provide support for airway management and emergency care outside the operating theatre when required.

- Participate in ICU/HDU responsibilities if included in the hospital's scope of work.

G.

Operating Theatre

Responsibilities

- Coordinate daily OT lists with orthopaedic surgeons and the OT team.

- Ensure appropriate pre-operative clearance and anaesthesia preparedness.

- Promote efficient and safe functioning of the operating theatre.

- Report equipment deficiencies, drug shortages or safety concerns promptly.

- Maintain required anaesthesia registers, records and documentation.

H. Quality, Compliance & Professional Responsibilities

- Maintain accurate and timely clinical documentation.

- Follow applicable NMC regulations, professional ethics and hospital policies.

- Participate in clinical audits, quality-improvement activities and morbidity/mortality reviews as applicable.

- Maintain confidentiality of patient information.

- Participate in continuing medical education and career development.

- Work collaboratively with surgeons, physicians, nursing staff and hospital administration.





- Provide teaching/training to OT and nursing staff on relevant emergency and anaesthesia-related protocols, where required.

1. Qualifications

Essential:

- MBBS from a recognised medical institution.

- MD/DNB in Anaesthesiology or an equivalent recognised postgraduate qualification.

- Valid registration with the appropriate medical regulatory authority.

- Appropriate experience in administering anaesthesia independently, as required by the hospital.

Desirable:

- Experience in orthopaedic and trauma anaesthesia.

- Experience with regional/neuraxial anaesthesia and peripheral nerve blocks.

- Experience in post-operative pain management.

- Training/experience in critical care, ATLS/ACLS/BLS or equivalent emergency-care training.

The NMC lists MD in Anaesthesia/Anaesthesiology among recognised postgraduate specialties and maintains competency-based postgraduate curricula.

1. Working Conditions

- Regular operating-theatre duties.

- May include emergency and on-call duties depending on hospital requirements.

- May be required to provide anaesthesia for trauma and urgent orthopaedic procedures.

- Working hours and on-call arrangements will be as per hospital policy and appointment terms.
