Job Summary

We are looking for a responsible and compassionate Nurse Coordinator to coordinate and manage home healthcare services for patients. The Nurse Coordinator will act as a link between patients, nurses, doctors, caregivers, and the operations team to ensure smooth, safe, and high-quality patient care.

Key Responsibilities

- Coordinate home nursing services and ensure timely deployment of nurses and caregivers.
- Assess patient care requirements and coordinate appropriate healthcare services.
- Communicate with patients and their families regarding care plans, schedules, and service requirements.
- Coordinate with nurses, doctors, physiotherapists, and other healthcare professionals.
- Monitor the quality and continuity of patient care provided at home.
- Follow up with patients and families regularly to understand their concerns and requirements.
- Manage nurse/caregiver schedules, attendance, and duty assignments.
- Handle patient complaints and coordinate appropriate solutions.
- Maintain accurate patient records, care plans, visit reports, and other clinical documentation.
- Ensure nurses follow proper clinical procedures, infection-control practices, and organizational protocols.




- Conduct regular follow-ups and escalate changes in the patient's condition to the appropriate medical team.
- Support the operations team in maintaining smooth day-to-day home healthcare services.
- Ensure proper handover and communication between healthcare staff.
- Maintain confidentiality and professional standards at all times.

Eligibility

- GNM or B.Sc Nursing qualification from a recognized institution.
- Valid nursing registration.
- 1–3 years of nursing experience preferred.
- Experience in home healthcare, hospital, clinical coordination, or patient care coordination will be an advantage.
- Good communication and interpersonal skills.
- Strong coordination and problem-solving abilities.
- Ability to communicate effectively with patients, families, and healthcare professionals.
- Basic computer knowledge and documentation skills.
- Willingness to travel for patient visits when required.
