Key Responsibilities:

1. Field Operations & Outreach

· Conduct daily field visits across assigned villages/blocks.

· Engage with SHGs, MRs, RMPs, and local stakeholders to strengthen healthcare referral networks.

· Mobilize patients for in-clinic consultations (eye, maternal health, sugar, general medicine, etc.).

· Identify and convert patients in the community for follow-up care and diagnostic services.

2. CHW Management & Capacity Building

· Monitor and support CHWs daily to ensure achievement of field targets.

· Conduct on-ground handholding, mentoring, and performance tracking of CHWs.

· Ensure CHWs are following program SOPs and are equipped with necessary tools and supplies.

3. Program Monitoring & Reporting

· Maintain detailed field visit reports, patient conversion data, and CHW performance sheets.

· Submit daily/weekly reports to the Program Coordinator.

· Identify gaps and suggest actionable improvements in field processes.

4. Community & Stakeholder Engagement

· Build strong rapport with village-level influencers, RMPs, and local health advocates.

· Assist in organizing community awareness meetings, camps, and outreach activities.

· Act as a bridge between the program team and field stakeholders to ensure smooth implementation.

Key Requirements:

· Minimum 2–4 years of field experience with NGOs, health programs, SHGs, or rural health stakeholders.

· Experience working with CHWs,



MRs, or RMPs is strongly preferred.

· Must be comfortable with daily field travel and managing decentralized field teams.

· Basic skills in data collection, mobile app or Google Sheets reporting preferred.

· Passionate about community health, patient care, and rural health transformation.

· Strong communication skills in local language (e.g., Bengali/Hindi) and interpersonal skills.

· Self-motivated, proactive, and result-oriented with solid problem-solving abilities.

Educational Qualification:

· Bachelor’s degree in Social Work, Rural Development, Public Health, or related field.

· Candidates with extensive field experience and a strong community network may be considered.

· Compensation:

· As per industry standards and based on experience.

· Travel allowance and mobile reimbursements applicable.

Pay: ₹15,000.00 - ₹20,000.00 per month
