Job Summary

We are looking for an experienced Medical Coder – Home Health with robust knowledge of home health coding and healthcare documentation. The ideal candidate should be CPC certified and have 4–6 years of relevant medical coding experience.

Key Responsibilities

- Review and analyze home health clinical documentation for accurate coding.
- Assign appropriate ICD-10-CM diagnosis codes based on physician and clinical documentation.
- Ensure accurate coding of diagnoses, conditions, and services provided in the home health setting.
- Review OASIS documentation,



Plan of Care (POC), physician orders, and clinical notes for coding accuracy.
- Apply home health coding guidelines and payer-specific requirements.
- Identify documentation gaps and communicate with relevant teams for clarification.
- Ensure coding accuracy, completeness, and compliance with applicable guidelines.
- Perform quality checks and maintain required productivity and accuracy standards.
- Stay updated on changes to ICD-10-CM, CMS, Medicare, and home health coding guidelines.
