Patient Care Coordinator

We are looking for a Patient Care Coordinator to manage patient communication, lead follow-ups, and coordinate healthcare services. The ideal candidate should have solid communication skills, good follow-up abilities, and working knowledge of CRM and Excel.

Key Responsibilities

- Connect with patients and leads through calls and other communication channels.
- Understand patient requirements and provide relevant information.
- Follow up with leads and patients to ensure timely coordination.
- Coordinate with internal teams for smooth service delivery.
- Maintain and update patient/lead data in CRM and Excel.
- Ensure a positive and seamless patient experience.

Requirements

- Excellent verbal and written communication skills.
- Good knowledge of MS Excel and CRM tools.
- Strong follow-up and coordination skills.
- Customer service or healthcare experience will be an advantage.
- Proactive, organized, and patient-focused approach.
