Will be responsible to receive the patients and to ensure that registration is done for current patients.
MSW will be responsible for collecting demographic details of the patients through the Registration form.
The MSW will also have to capture details of the referring doctor and patients family doctor details.
Provide psychosocial assessment and counselling support to patients and families.
Assist patients in addressing social, emotional, financial, and family-related concerns.
Coordinate with psychiatrists, psychologists, nurses, and other healthcare specialists.
Conduct patient and family interviews and maintain appropriate case records.
Support treatment adherence, rehabilitation, and community reintegration.
Identify social support systems and connect patients with relevant welfare schemes and community resources.
Assist with discharge planning and appropriate referrals.

Preferred: Specialization or experience in Medical & Psychiatric Social Work / Mental Health.
