Face-to-Face Counselling: Provide in-person counselling sessions to patients in hospital
Collaboration with Doctors: Regularly meet with doctors to update them on patients' progress and
treatment plans.
Telephonic Counselling: Offer telephonic counselling to patients where face-to-face meetings are not
feasible, following written patient consent.
Patient Training: Conduct patient training programs for 5 days or more, specifically focusing on
managing ITP.
Follow-Up Tracking: Monitor patient follow-up visits at key intervals, including 0-3 months and 3-6
months post-treatment.
Transfer Set Maintenance: Ensure transfer sets are changed every 6 months for applicable patients.
Patient Education Programs: Organize and conduct educational programs to improve patient
knowledge and health management.
End-to-End Patient Support: Provide comprehensive support to patients throughout their treatment
journey.
Documentation and Reporting:
Capture relevant data and report in the specified format within
agreed timeframes.
Follow-Up Management: Maintain a transparent follow-up process to ensure continuous patient care.
Qualification & Skills:
BSC-Nursing/GNM-Nursing/BSC in Dialysis technologies/Diploma in Dialysis technician.
Experience in Counselling 0-1 years
Good communication in local language of respective city and English
Compassion and empathy skills required
Good comprehension skills
Ready to travel Out stations as well as within the city & should know areas of the city
Should have good clinical knowledge about Peritoneal dialysis
Pay: ₹30,000.00 - ₹35,000.00 per month
