About the RoleThe Family-Based Care Coordinator/Social Worker will work with children and families who have transitioned from a residential care institute to family-based care. The role focuses on ensuring the child's safety, education, health, well-being, protection, and continued family support through regular follow-up and case management.

Key Responsibilities

- Conduct regular home visits and follow up with children and families.

- Assess children's and families' needs and prepare individual support plans.

- Monitor children's education, health, safety, and overall well-being.

- Provide basic counseling and emotional support to children and caregivers.

- Identify child protection concerns and make appropriate referrals.

- Support families in accessing government schemes, entitlements, education, health, and other services.

- Coordinate with schools, colleges, hospitals, government departments, and community resources.

- Conduct awareness sessions on child rights, education, health, protection,



and family well-being.

- Maintain case files, home-visit records, MIS, and other program documentation.

- Prepare monthly reports and share regular updates with the supervisor.

- Coordinate with CCI staff, Social Mobilizers, and other Need Base India programs.

Qualifications & Experience

- Bachelor’s degree in social work, Sociology, Psychology, Child Development, or a related field.

- Experience working with children, families, or communities is preferred.

- Knowledge of child rights and child protection is an advantage.

- Positive communication, documentation, and coordination skills.

- Basic computer knowledge.

- Willingness to travel for field visits.

Pay: ₹18,000.00 - ₹20,000.00 per month
