Job Summary

We are looking for an experienced Medical Coder – Home Health with solid knowledge of home health coding and healthcare documentation. The ideal candidate should be CPC certified and have 4–6 years of relevant medical coding experience.

Key Responsibilities
Review and analyze home health clinical documentation for accurate coding.
Assign appropriate ICD-10-CM diagnosis codes based on physician and clinical documentation.
Ensure accurate coding of diagnoses, conditions, and services provided in the home health setting.
Review OASIS documentation,



Plan of Care (POC), physician orders, and clinical notes for coding accuracy.
Apply home health coding guidelines and payer-specific requirements.
Identify documentation gaps and communicate with relevant teams for clarification.
Ensure coding accuracy, completeness, and compliance with applicable guidelines.
Perform quality checks and maintain required productivity and accuracy standards.
Stay updated on changes to ICD-10-CM, CMS, Medicare, and home health coding guidelines.
