JOB DESCRIPTION – MRD IN-CHARGE
Maintain and manage all Medical Records Department (MRD) activities efficiently.
Ensure proper collection, verification, filing, indexing, and preservation of patient records.
Maintain both physical and electronic medical records accurately and systematically.
Ensure timely availability of patient records for doctors, departments, billing, insurance and audits.
Maintain confidentiality and security of all patient information and medical records.
Ensure proper discharge summary, case sheet and investigation report filing and completeness.
Coordinate with clinical and administrative departments for missing or incomplete records.
Maintain MRD registers, statistics, reports and daily/monthly records as required.
Handle requests for medical records from authorized persons as per hospital policy.
Ensure proper retention,
storage and disposal of records according to hospital rules.
Support insurance/TPA, legal, accreditation and audit requirements by providing records when authorized.
Monitor the work of MRD staff and allocate duties appropriately.
Ensure proper data entry and updating of patient records in the hospital software/system.
Prepare and submit monthly MRD reports and statistics to the management.
Identify discrepancies, maintain record quality, and report key issues to the concerned authority.
Ensure compliance with hospital policies, documentation standards and patient confidentiality requirements.
