Roles & responsibilities:

A. Direct patient care

- Report to the Supervisor/In-chargeon duty.
- Be punctual for the duties.
- Complete special assignments like Inventory checking , dressing trolley maintenance, crash cart inventory etc.
- Ensure that patient allocation is taken over based on patient acuity score which helps the prioritize the patient care.
- Check vital signs and inform any variation to the Medical team, in-charge or Supervisor.
- Carry out the interventions as per doctors order.
- Prioritize the Nursing care based on the condition of the patients.
- Meet the basic hygenic needs of all patients like bath, mouth care, Catheter care etc.
- provide individualized care with comprehensive & holistic approach.
- Document the care given in the nurses record based on the documentation policy.
- Keep patient's files, assessment trays ready for doctor rounds, attend rounds and carry out the orders.
- Ensure to adhere to patient admission policy. Ensure room and bed reediness before patient arrive to the unit.
- On admission receive the patient and make him / her comfortable.

· Room Orientation to be given to the patients. · Initial assessment to be completed within 20 minutes of admission & reassessment in each shift.

· All patient care plans has to be documented & should be modified as per the condition of patients.

· Vital signs & pain assessment to be checked in every 4 hourly & as when required.

· All information to be exchanged & documented in each shift , between shift & during transfer between unit / departments.

· Dietary information to be given to the nutrition team during the day & F& B staff in the night.

· Insert peripheral venous catheters as and when required, if you are not getting the line in the first attempt call the senior staff/ supervisor for the assisting.

· Inform duty doctor/ consultant about the admission/ Transfer in/Transfer out cases.

· Ensure Close monitoring of vulnerable & patient on restrains.

· Ensure to do the assessment,



re-assessment in every shift for the patients for Pressure Ulcer , DVT and Fall risk etc.

- Change 2 hourly positions and back care for vulnerable patient's who all are high risk for developing pressure ulcer.
- Assist for Minor / major dressings of the patient.
- Adhere to sample collection policy & protocols strictly .
- Collect the lab reports on time and inform the concern duty doctors/ consultants.
- For any investigation or procedure, the patient should be prepared as per the consultants order and shifted as per hospital policy.
- Complete the nurse’s documents immediately after each procedure.
- Check pre -op patient for skin preparation, site marking, investigation reports, consents, pre--op check etc before shifting to OT/ cath lab.
- While transporting the patient to OT nurse should accompany the patient and ensure to give proper handover the OT nurse as per the policy.
- Any patient's problems to be informed to the In charge/Supervisor.
- On patient’s discharge, see that the discharge procedure is carried out as per the policy.
- Ensure that end of life care / Death care to be given with respect & dignity.
- All staff should know about all emergency codes & policies.
- To co-ordinate and work for the smooth functioning of the unit.

B . Infection control Practices · Ensure the cleanliness of unit & patient premises at all times.

· Infection control protocols to be carried out : like Proper Hand hygiene,

wearing PPE appropriately ,Use of hand sanitizer before & after touching the patients

& follow proper Biomedical waste segragation standards.





· Attend HIC training sessions & update the knowledge.

· Mention date and time of insertion of tubes & catheters.

· Inform supervisor and duty doctor to check the blood and enter in the respective progress notes.

· Assess Patient's IV Line frequently to prevent extravasations/ infiltration etc.

C-Training & Development :

· Attend 6 -10 hours of CNE training sessions every month.

· Attend departmental hands on training .

· Take responsibility to meet Nurse educator for individualized training sessionsas per requirements.

· Be proactive to clarify doubts to senior nurses , supervisors , Nurse educator etc. if there is any clarification required about the doctors orders, procedures, medication orders etc.

· Take initiative to do case presentation per the assignment given by the Nurse educator.

D-Medication Management:

· Indent the medication as per the written order.

- Check patient’s case file and give medication –

· Right drug · Right dose

· Right patient

· Right time

· Right route

· Right to refuse

· Right assessment

· Right Education

· Right Evaluation

· Right drug documentation

· Enter in the medication chart immediately after administering the drug and observe for any reaction

- Check for chemotherapy patients for chemo-plan as written by the respective consultants.
- Check for consent before chemo cycle.
- Check vitals every hourly & follow MEWS (Modified Early Warning Score) guideline, if there is any deviation inform duty doctor/consultant immediately and carry out the orders.
- Telephonic orders can be taken only at the time of emergency & it must be confirmed by another staff nurse or duty doctor and documented in the verbal order form and get it signed by consultant within 24hrs.
- Strict compliance to Blood/blood products transfusion policy.
