- Provide social and emotional support to residents and understand their individual needs.

- Conduct regular interactions with residents and monitor their social and emotional well-being.

- Assist in developing individual care and rehabilitation plans in coordination with the care team.

- Coordinate with family members and maintain regular communication regarding residents.

- Support residents in maintaining family and community relationships.

- Conduct counselling and supportive sessions as required.

- Maintain resident case files, assessments, progress notes, and other documentation.

- Coordinate with doctors, therapists, caregivers, and other professionals for comprehensive resident care.

- Support residents in accessing government schemes, advantages,



and other available services.

- Organise recreational, social, and community-based activities for residents.

- Identify social, behavioural, or family-related concerns and report them to the Manager.

- Assist the Manager in resolving resident and family concerns.

- Participate in awareness programmes, outreach activities, and community engagement initiatives.

- Maintain confidentiality and ensure residents are treated with dignity, respect, and compassion.
