Qualifications

Roles and Responsibilities Senior Pharmacist: Roles & Responsibilities

1. Prescription Review & Dispensing





Review prescriptions for correct medicine, dose, route, frequency, duration, duplication, contraindications, allergies, and potential drug interactions.

Ensure accurate dispensing and appropriate labeling of medications.

Clarify incomplete, illegible, unusual, or potentially unsafe prescriptions with the treating doctor before dispensing.

Give special attention to high-alert medicines, pediatric doses, renal/hepatic dose adjustments, and medicines with a narrow therapeutic index.

Ensure proper handling and documentation of controlled or restricted medicines according to applicable hospital policies and regulations.

2. Medication Safety

Promote the hospital's medication safety program and safe medication-use practices.

Identify, document, and report medication errors, near misses, and adverse drug reactions through the hospital's reporting system.

Participate in Root Cause Analysis (RCA) and implementation of Corrective and Preventive Actions (CAPA) following significant medication incidents.

Monitor look-alike/sound-alike (LASA) and high-alert medications and ensure appropriate segregation and warning labels.

Support implementation of safe storage, dispensing, administration, and monitoring practices.

3. Clinical Pharmacy Support

Provide medicine-related information to doctors, nurses, patients, and other healthcare professionals.

Review medication profiles, especially for patients receiving multiple or high-risk medications.

Support antimicrobial stewardship activities according to the hospital's antibiotic policy.

4. Pharmacy Inventory Management

Maintain appropriate stock levels of medicines and pharmacy consumables.

Monitor minimum/maximum stock, reorder levels, fast-moving, slow-moving, and non-moving items.

Ensure proper implementation of FEFO, First Expiry First Out.

Conduct regular checks for near-expiry, expired, damaged, recalled, and quarantined medicines.

Coordinate with purchase, stores, vendors, and clinical departments to prevent critical stock-outs.

Investigate significant physical-stock versus system-stock discrepancies.

Help control wastage and inventory carrying costs without compromising patient availability.

5. Storage & Cold-Chain Management

Ensure medicines are stored according to manufacturer requirements and hospital policy.

Monitor and document pharmacy and refrigerator temperatures.

Ensure cold-chain medicines are maintained within their specified temperature range.



For many refrigerated pharmaceutical products this is typically 2C to 8C, while the manufacturer's requirements for the individual product should always be followed.

Take appropriate action when temperature excursions occur.

Ensure proper segregation of expired, recalled, damaged, returned, hazardous, and other specially controlled medicines.

6. Team Supervision A Senior Pharmacist normally has greater supervisory responsibility than a staff pharmacist.

Allocate duties and shifts to pharmacists, pharmacy technicians, and support staff as applicable.

Supervise dispensing and pharmacy operations during the assigned shift.

Train current pharmacists and technicians on hospital pharmacy procedures.

Importance of medication adherence For discharge patients, help ensure they understand changes between their previous medicines and the discharge prescription.

9. Emergency & Critical-Care Support

Ensure availability of emergency and life-saving medications.

Periodically check emergency/crash-cart medicines for quantity, expiry, storage, and completeness according to hospital policy.

Facilitate urgent medication supply to ICU, emergency department, operating theatre, and wards.

Maintain an escalation mechanism for critical medication stock-outs.

10. Documentation & Reporting

Prepare or review periodic reports such as:

Medication error and near-miss reports

Adverse drug reaction reports
