Nurse Manager – Home Visits

Role Summary The Nurse Manager – Home Visits will be responsible for coordinating and providing comprehensive home-based healthcare to elderly patients. The role involves accompanying doctors on home visits, independently providing nursing care at patients’ homes, implementing care plans, coordinating investigations and medications, following up patients, and serving as the single point of contact for patients and their families.

The Nurse Manager is expected to work independently in the community and ensure continuity, quality and timely delivery of care.

Key Responsibilities

1. Home Visits & Independent Nursing Care

- Accompany doctors during scheduled home visits and assist with clinical assessment and care planning.
- Independently provide nursing care at patients’ homes as clinically appropriate.
- Perform procedures such as:
- Major and minor wound dressings
- Urinary catheterisation and catheter care
- Ryle’s tube/feeding tube care
- Injections and medication administration as prescribed
- Pressure sore management
- Tracheostomy and other specialised nursing care, where trained and appropriate
- Monitor patients and identify changes in their clinical condition.
- Implement and regularly review patient-specific care plans under medical guidance.
- Coordinate additional nursing, physiotherapy, laboratory and other home-care services as required.

2. Care Coordination & Case Follow-up





- Serve as the single point of contact for assigned elderly patients and their families.
- Follow up patients regularly and ensure that planned interventions are completed.
- Coordinate with doctors regarding changes in the patient's condition.
- Ensure continuity of care between doctor visits.
- Maintain accurate patient records, care plans and follow-up documentation.

3. Laboratory, Pharmacy & Billing

- Raise laboratory investigation requests and coordinate sample collection as prescribed.
- Coordinate pharmacy requirements and prescriptions as directed by the doctor.
- Follow up investigation reports and communicate relevant findings to the treating team.
- Raise and coordinate billing for home healthcare services.
- Ensure timely documentation of all services provided.

4. 24×7 Family & Patient Support

- Attend calls from patients and their families 24×7 as part of the assigned responsibility.
- Provide appropriate nursing guidance within the scope of practice.
- Recognise emergencies and deterioration in condition and promptly escalate to the doctor/Medical Director.
- Coordinate emergency nursing visits, hospital transfers or other interventions when required.
- Maintain professional, compassionate and reassuring communication with families.

5. Community Healthcare

- Participate in and coordinate community vaccination drives.




- Support health screening and preventive healthcare programmes for senior citizens.
- Participate in community outreach and elder-care awareness programmes.

Qualifications & Experience

- Registered Nurse (RN/RM) with valid nursing registration.
- Minimum 3 years of nursing experience preferred.
- Experience in home healthcare, geriatrics, community nursing, wound care or hospital nursing will be an advantage.
- Freshers with good clinical knowledge and practical nursing skills may also apply.

Additional Requirements

- Must be comfortable travelling independently to patients’ homes across Coimbatore.
- Must know how to ride a two-wheeler.
- Having a personal two-wheeler is preferred.
- Should be comfortable performing nursing procedures independently in a home-care environment.
- Willingness to take responsibility for patients and coordinate their care independently.
