Eligibility : Doctor Of Pharmacy (Pharm D) or M. Pharma (Pharmacology/Pharmacy Practice).

Daily Tasks

1. Auditing all the in-patient order for drug name, strength, formulation, dosage and duration of therapy, therapeutic duplication and to check for possible drug interaction, if any.
2. Checking the physician’s drug orders for appropriateness in the patient care areas and rectification of any medication error observed with appropriate documentation as per hospital protocol.
3. Check for the medication errors daily and document, if any.
4. For clarification check the patient record at the ward / ICU discuss with Clinical Pharmacologist.
5. In case any non formulary drugs are indented inform to the concerned doctor and substitute it with the drugs given in the formulary.
6.



Coordinate with the Supervisor IP for issue of substitute in such cases.
7. Responsible for checking the drugs in impress stock, crash cart, disaster kits and ambulance regularly.
8. Responsible for documentation of restricted antibiotics and report it to Clinical Pharmacologist.
9. Monitoring of adverse drug reaction.
10. Query handling as raised by different healthcare professionals.
11. JCI/NABH Audit.

Pay: ₹21,000.00 - ₹25,000.00 per month
