Telemedicine & Digital Healthcare Access Initiative – Chennai

Role Purpose

Community Health Workers will be the first point of contact for beneficiaries. They will conduct community awareness, mobilise patients, support registration and consent, record symptoms and medical history, conduct approved basic health tests, assist during teleconsultation, guide patients for medicine access and referral, and conduct follow-up calls / visits.

Qualification & Experience

Minimum 12th pass; graduate preferred. Local candidates with experience in community mobilisation, health outreach, field work, patient support, NGO projects, public health campaigns or digital data collection will be preferred.

Key Responsibilities

- Conduct community awareness and mobilisation activities.
- Identify eligible beneficiaries from underserved communities.
- Support patient registration, consent and demographic data collection.
- Record symptoms, medical history, chronic conditions and risk factors.
- Conduct only approved non-invasive / point-of-care basic tests as per protocol.
- Record BP, blood glucose, SpO₂, temperature, pulse rate, respiratory rate, weight / BMI and single-lead ECG readings, wherever applicable.




- Upload / record test readings in the telemedicine platform or approved reporting format.
- Support patients during live teleconsultation with doctors.
- Guide patients to local pharmacies, Jan Aushadhi Centres or government facilities as advised.
- Escalate abnormal readings, red-flag symptoms or emergency signs to the doctor / referral facility.
- Conduct follow-up calls / visits for chronic, high-risk and referred patients.
- Maintain daily field records, attendance, follow-up records and referral status.
- Support non-clinical CSR employee volunteering activities where required.

Clinical Scope & Safeguard SIGNIFICANT CLINICAL BOUNDARY Community Health Workers will not diagnose diseases, prescribe medicines or change prescriptions. Medical interpretation, diagnosis, treatment advice and referral decisions will remain the responsibility of registered medical practitioners.

- Daily mobilisation and registration records.
- Patient consent and basic profile documentation.
- Basic test records.
- Teleconsultation support records.
- Referral and follow-up tracker.
- Daily field report submitted to Telemedicine Operator / Project Lead.

Pay: ₹18,000.00 - ₹23,000.00 per month
