Position: Client Finance Specialist
Total Compensation: ₱42,500 – ₱49,000 PHP per month
Bonus : Up to 10% of patient responsibility collected
The Client Finance Specialist owns the patient-responsibility side of Akua's revenue cycle. This role is responsible for identifying, communicating, and collecting the portion of treatment costs owed directly by clients and their families — copays, deductibles, coinsurance, and self-pay balances — across Akua's residential, outpatient, and sober-living programs. The Specialist works closely with Admissions, Utilization Review, and the RCM billing team to ensure every client has a clear financial agreement in place before or at the time of admission, and that patient balances are collected accurately, respectfully, and on time.
This is a remote, client-facing finance role supporting a U.S. behavioral health provider. The right candidate is comfortable discussing money with American families in a difficult moment by phone, is disciplined about follow-up, and understands that timely, compassionate collection of patient responsibility protects both the client's access to care and the financial health of the organization.
Key Responsibilities
Financial counseling and agreements
- Review verification of benefits (VOB) results to determine each client's expected out-of-pocket responsibility, including deductible remaining, copay, coinsurance, and out-of-pocket maximum.
- Prepare and clearly explain patient responsibility estimates to clients and financial guarantors by phone and email before or at admission; send financial responsibility agreements and payment authorizations for e-signature and confirm they are executed before intake, coordinating with on-site Admissions staff where an in-person signature is needed.
- Structure and document payment plans within company policy for clients who cannot pay in full at admission, and set up recurring card or ACH payments where appropriate.
- Provide Positive Faith Estimates to self-pay and uninsured clients in compliance with the No Surprises Act.
Collections and account management
- Collect copays, deductibles, coinsurance, and self-pay balances at admission and throughout the episode of care, including for step-downs between levels of care, using phone, email, text,
and secure online payment links.
- Reconcile patient responsibility against payer explanations of benefits (EOBs) and remittances after adjudication; issue accurate patient statements and adjust balances for payer reprocessing, write-offs, or refunds.
- Manage an active patient-balance work queue: prioritize by balance and aging, execute outreach by phone, email, and text, and document every contact and promise-to-pay in the billing system.
- Process payments, post them accurately to the correct client account and date of service, and reconcile daily deposits against the merchant processor and the general ledger.
- Identify accounts requiring escalation, financial hardship review, or referral to outside collections, and prepare the supporting documentation for management approval.
- Process client refunds and credit balances in accordance with policy and payer requirements.
Reporting and collaboration
- Maintain and report weekly on patient AR: balances outstanding by aging bucket, collections against target, payment-plan compliance, and accounts at risk.
- Partner with Admissions to ensure financial clearance is completed before intake and with Utilization Review to anticipate changes in authorized level of care that affect patient cost share.
- Coordinate with the RCM billing and payer-collections team so that patient and payer balances are never pursued in conflict with one another.
- Respond to client and family billing inquiries and disputes promptly and professionally, resolving issues at first contact whenever possible.
- Recommend improvements to financial-clearance workflows, scripts, and policies based on collection results and client feedback.
Qualifications
Required
- 2+ years of experience in U.S. healthcare patient financial services, patient collections, or medical billing — in-house or with an RCM/BPO provider — with direct responsibility for collecting patient balances.
- Working knowledge of commercial insurance benefits (deductibles, copays, coinsurance, out-of-pocket maximums, in-network vs. out-of-network) and the ability to explain them plainly to a layperson.
- Ability to read and reconcile EOBs and remittance advice to a patient balance.
- Demonstrated success collecting balances by phone from U.S. patients while maintaining a respectful, empathetic tone; excellent spoken and written English with a clear, professional phone manner.
- Proficiency with Microsoft Excel and a healthcare billing or EMR system; accurate data entry and strong attention to detail.
- Understanding of HIPAA and the confidentiality requirements specific to substance use disorder treatment records (42 CFR Part 2).
- Ability to work U.S. Pacific Time business hours consistently.
- Dedicated, private home workspace with reliable high-speed internet, backup power/connectivity, and a computer that meets company security requirements (a company-managed device or VDI may be provided).
Preferred
- Experience supporting U.S. behavioral health or substance use disorder treatment providers.
- Prior remote contractor experience with a U.S. healthcare client.
- Familiarity with Kipu or a comparable behavioral health EMR and with merchant payment processing platforms.
- Experience with Medi-Cal, TRICARE, and county-funded programs and their patient cost-share rules.
- Bilingual English/Spanish.
- Associate's or bachelor's degree in business, finance, healthcare administration, or a related field.
