- Documentation should be done (in SAP) as per the physician’s order and not authorized to do any documentation without the knowledge of the physician.
- Documentation of the outpatient care in SAP

2.1. Transcribing / entering the outpatient management details in the SAP as per the physician conversation, dictated file or written format.

2.2. Transcribing the dictated below mentioned reports

2.2.1. Procedure / operation note

2.2.2. Discharge summary

2.2.3. Medical certificate

2.2.4. Consultation reports

- Work edited and proof read to ensure accuracy
- Spelling and grammar errors are minimized
- Patient information identified on all reports.
- Patient confidentiality maintained at all times.
- Participation in quality improvement activities.

8.Ensure TAT is maintained at all time
