Key Responsibilities & Duties

1. Patient Care & Assessment

- Assist patients with activities of daily living (hygiene, feeding, mobility) when necessary.
- Recognize and respond promptly to changes in patient condition or emergencies.
- Daily Living Support (ADLs): Assist with personal hygiene, bathing, dressing, grooming, and toileting.
- Companionship & Emotional Support: Engage in meaningful conversations, reading, light activities, and accompany them on short walks to prevent isolation.
- Understand dietary needs: Provide meals tailored to the client's dietary requirements and assist with feeding if necessary.
- Medication Reminders: Remind the client to take prescribed medications on time (non-administration).
- Errands & Escort: Accompany clients to medical appointments, grocery shopping, or social visits.
- Status Reporting: Observe and report any changes in physical health, mental awareness, or behavior to family members or the care coordinator.

2. Medication Administration & Treatments

- Administer oral, intravenous (IV), intramuscular (IM), and subcutaneous medications accurately following the 5 Rights of Medication Safety (Right Patient, Drug, Dose, Route, Time).
- Manage intravenous lines, blood transfusions, wound care, dressings,



and catheterizations.
- Operate medical equipment such as ECG machines, suction apparatus, pulse oximeters, and IV infusion pumps.

3. Documentation & Reporting

- Maintain accurate, real-time clinical notes, medication administration records (MAR), and handover charts.
- Report critical lab values and significant changes in patient status immediately to attending physicians.
- Ensure smooth handovers at shift changes using standard communication protocols (e.g., SBAR).

4. Infection Control & Patient Safety

- Strictly follow hospital infection control policies, hand hygiene practices, and standard precautions.
- Maintain aseptic techniques during procedures and proper disposal of biomedical waste (BMW management).
- Ensure patient safety protocols (e.g., fall prevention, pressure ulcer care, restraint policies).

5. Collaboration & Education

- Coordinate with multidisciplinary teams (doctors, lab technicians, physiotherapists, dietitians) for holistic patient care.
- Educate patients and their families regarding discharge plans, home care procedures, and medication compliance.Role & responsibilities
