About the Role

We are looking for a Patient Reactivation Executive / Lapse Package Coordinator to connect with patients whose treatment packages are inactive or have lapsed.

The role involves regular follow-ups, understanding patient concerns, coordinating with the relevant centre teams, and assisting patients in restarting their treatment journey.

Key Responsibilities
Follow up with patients having lapsed or inactive treatment packages.




Understand the reasons for treatment discontinuation and address patient concerns professionally.
Make regular outbound calls and maintain proper follow-up records.
Coordinate with doctors, counsellors and centre teams as required.
Schedule consultations and appointments for interested patients.
Maintain accurate patient information and update follow-up status in CRM/HMS.
Ensure timely follow-ups and work towards assigned patient reactivation targets.
Escalate clinical or service-related concerns to the co
