Role Description The Psychiatric Social Worker is a full-time, on-site role based in Tricity. This role involves conducting psychosocial assessments, developing and implementing individualized treatment and care plans, and providing therapeutic interventions to individuals and families.

The Psychiatric Social

Worker will coordinate case management activities, including referrals, discharge planning, and linkage to community resources and support services. Daily responsibilities include collaborating with psychiatrists, psychologists, nurses, and other team members to ensure integrated care, maintaining accurate and timely documentation, and advocating for clients’ rights and access to services.



The role also includes offering psychoeducation to patients and caregivers and participating in multidisciplinary meetings and quality improvement initiatives.

Qualifications

- Candidates should possess robust Social Work skills, including psychosocial assessment, counseling, and advocacy.
- Candidates should possess skills in Mental Health and Behavioral Health, with experience supporting individuals with diverse psychiatric conditions.
- Candidates should possess Treatment Planning skills, including developing, implementing, and monitoring individualized care plans.
- Candidates should possess Case Management skills, including coordination of services, referrals, and follow-up with community resources.
- Master’s degree in Social Work or equivalent relevant discipline, with appropriate licensure or eligibility as required by local regulations.
- Experience in psychiatric or clinical mental health settings, preferably inpatient or outpatient facilities.
- Strong communication, interpersonal, and crisis intervention skills, with the ability to work effectively in a multidisciplinary team.
- Ability to maintain professional boundaries, uphold confidentiality, and demonstrate cultural sensitivity and trauma-informed practice.
