About the Role

Icosano Home Health Care Pvt. Ltd. is looking for an experienced and responsible Nursing Manager to lead our nursing operations, with a primary focus on ICU-at-home setup, patient assessment, clinical coordination, and nursing team management.

The candidate will be responsible for assessing patients before onboarding, understanding their clinical and nursing requirements, coordinating ICU setup at home, and ensuring that appropriate nursing resources and equipment are arranged for secure and quality patient care.

Key Responsibilities1. Patient Assessment & Clinical Evaluation

- Conduct detailed initial patient assessments before starting home healthcare services.
- Review patient history, diagnosis, current treatment and clinical condition.
- Assess patients requiring ICU setup, critical care, post-operative care and long-term nursing care.
- Identify nursing requirements, monitoring needs, mobility limitations, wound care requirements and other clinical needs.
- Prepare an appropriate home-care nursing plan based on the patient's condition.
- Coordinate with treating doctors and hospitals wherever clarification or clinical inputs are required.
- Conduct reassessment of patients whenever there is a significant change in clinical condition.

1. ICU-at-Home Setup

- Assess the patient's home environment and determine the requirements for an ICU-at-home setup.
- Coordinate with the equipment and operations team for arrangement of:
- Hospital/ICU bed
- Oxygen concentrator/cylinder
- Suction machine
- Patient monitor
- Ventilator/BiPAP/CPAP, wherever prescribed
- Infusion/syringe pumps
- Other required medical equipment
- Ensure that equipment and clinical resources are available before patient onboarding.
- Coordinate with the nursing team for proper implementation of the care plan.
- Ensure appropriate infection-control and patient-safety practices at the patient's home.

1. Nursing Team Management

- Supervise and manage nurses, ICU nurses and nursing coordinators.




- Allocate nurses according to patient requirements and clinical complexity.
- Prepare and monitor nursing duty rosters and deployment plans.
- Ensure adequate staffing and timely replacement in case of absenteeism.
- Monitor nursing performance, discipline and skilled conduct.
- Conduct regular meetings and clinical reviews with nursing staff.
- Identify training requirements and arrange clinical/skill-based training.

1. Clinical Quality & Patient Safety

- Ensure compliance with Icosano's nursing SOPs and clinical protocols.
- Monitor medication administration, vital monitoring, documentation and nursing procedures.
- Ensure proper maintenance of patient records and nursing reports.
- Monitor incidents, clinical escalations and patient complaints.
- Take immediate action in case of clinical deterioration and escalate to the appropriate doctor/emergency service.
- Promote infection prevention and control practices during home visits and patient care.

1. Hospital & Doctor Coordination

- Coordinate with treating consultants, hospitals and doctors regarding patient requirements.
- Understand discharge summaries, treatment plans and nursing instructions.
- Ensure smooth transition of patients from hospital to home healthcare.
- Maintain skilled relationships with hospital nursing teams and doctors.
- Participate in clinical discussions related to complex patients whenever required.

1. Family & Patient Coordination

- Explain the proposed home nursing plan to patients and family members.
- Educate caregivers/family members regarding basic patient care and safety.
- Address clinical concerns raised by patients and their families.
- Ensure timely escalation of patient-related concerns.
- Maintain a professional and compassionate approach while dealing with critically ill patients and their families.

1. Documentation & Reporting

- Maintain patient assessment and care-plan records.
- Review nursing documentation and daily patient reports.
- Maintain records of patient visits,



clinical escalations and incidents.
- Prepare regular nursing department reports for management.
- Monitor patient feedback and service quality.

Key Skills Required
- Strong knowledge of ICU and critical care nursing.
- Excellent patient assessment and clinical decision-making skills.
- Experience in handling ventilated and critically ill patients.
- Knowledge of ICU equipment and home ICU setup requirements.
- Strong nursing team management and leadership skills.
- Good communication and coordination skills.
- Ability to communicate effectively with doctors, patients and family members.
- Strong documentation and reporting skills.
- Ability to handle emergency situations calmly and effectively.
- Good understanding of infection control and patient safety practices.

Preferred Candidate Profile
- B.Sc Nursing / GNM from a recognized institution.
- Valid nursing registration with the applicable State Nursing Council.
- 5–10 years of relevant nursing experience, preferably in ICU, critical care, hospital or home healthcare.
- Experience in home ICU setup / transition from hospital to home will be an advantage.
- Candidates with experience in ICU, HDU, CCU, ventilator care, neurology, pulmonary, oncology or post-operative critical care will be preferred.
- Previous experience in nursing supervision or nursing management will be an advantage.
