The Transitional Care Staff Nurse coordinates the clinical care of patients moving from the hospital to home . Key responsibilities include performing post-acute clinical assessments, conducting vital medication reconciliation, and administering treatments like wound care or IV therapy. The nurse delivers direct patient and caregiver education on symptom management, monitors patients via post-discharge follow-up calls, and collaborates closely with the dedicated discharge planning team.Qualifications require an active Registered Nurse (RN) license and a nursing degree . Freshers and newly licensed graduates are welcome to apply, as comprehensive onboarding and clinical mentorship will be provided. Solid communication skills, adaptability, and a compassionate attitude are essential for success in this role.
Pay: ₹18,000.00 - ₹25,000.00 per month
