Role Description The HCC Coder is a full-time, on-site role located in the Greater Kolkata Area, responsible for accurately assigning Hierarchical Condition Category (HCC) codes based on clinical documentation and established coding guidelines. Day-to-day tasks include reviewing medical records, interpreting provider documentation, applying appropriate ICD codes to reflect patient risk and severity, and ensuring compliance with regulatory standards and payer requirements.
Qualifications
- Coding Experience and Medical Coding skills relevant to HCC, ICD, and risk adjustment methodologies.
- Knowledge of Medical Terminology and Health Information Management principles to interpret and code clinical documentation accurately.
- Professional credentials such as RHIT or equivalent coding certification (e.g., CCS, CPC, CRC) are strongly preferred.
- Familiarity with payer guidelines, risk adjustment models, and regulatory requirements for compliant coding.
- Robust attention to detail, analytical skills, and ability to work with electronic health records and coding software.
- Effective communication skills for collaborating with providers, auditors, and billing teams.
- Diploma or degree in Health Information Management, Medical Coding, or a related healthcare field.
- Experience in HCC/Risk Adjustment coding or outpatient/inpatient diagnosis coding is an advantage.
