: Patient navigation, registration, referrals, follow-up and support for patients and families. Conduct social, family and socioeconomic assessments and assist with welfare/concession services. Organise and support outreach clinics, medical camps, wellness programmes and community health activities.

Conduct household surveys, census/enumeration, home visits and maintain community records. Mobilise and coordinate community members, Health Aides, Family Care Volunteers and local organisations. Identify vulnerable individuals/households and facilitate appropriate health and social support.

Support community development projects, research activities, student/intern field visits and data collection. Maintain documentation and reports, ensure confidentiality and work collaboratively with RUHSA team.

Job Preference: Those who have completed 4 years of experience as Jr. Field Worker (Term Appointment) or equivalent only need to apply.
