Duration of Position: 05 Months

Location: Project Facilities in Ahmedabad and Aravalli districts,

The Field Officers will be deployed across the following six participating health facilities;

Brief On The Organization

Indian Institute of Public Health Gandhinagar (IIPHG) is a University established under IIPHG Act, 2015 of Gujarat State. It offers courses in the field of Public Health. It also undertakes various research projects in the field of Public Health. The university is established by Public Health Foundation of India (PHFI) with support from the Government of Gujarat, Department of Health & Family Welfare.

Project Brief

IIPHG proposes to implement a Facility-based Intensified TB Case Finding Project across six public health facilities in Gujarat. The project will strengthen early identification of TB through systematic screening of patients attending selected OPDs and IPDs, vulnerability assessment, CXR facilitation, diagnostic evaluation, patient navigation and follow-up, and linkage with treatment, NTEP and TPT services.

The project will also strengthen facility-level implementation through standardized SOPs and workflows, healthcare-provider upskilling, Project MIS-based patient tracking and performance monitoring, supportive supervision, and documentation of operational learnings for potential institutionalization and replication.

Deliverables The Field Officer will support day-to-day facility-level implementation of the intensified TB case-finding pathway, including patient enrolment and registration, TB screening, patient navigation, CXR and diagnostic coordination, follow-up, data collection and linkage with facility/NTEP services.

Key Responsibilities

- Patient Enrolment and TB Screening

- Approach and facilitate enrolment of eligible patients attending identified OPD/IPD screening points.

- Register patients according to the approved project workflow.

- Conduct standardized 10-Symptom (10-S) screening and vulnerability assessment using approved project tools.

- Ensure accurate and complete capture of required patient information.

- Identify patients requiring CXR or further evaluation according to the approved screening pathway.

- Support screening of eligible patients identified through other facility entry points, including CXR/Radiology, where applicable.

- Patient Navigation and CXR Coordination

- Guide eligible patients through the facility-based TB screening and diagnostic pathway.

- Facilitate movement of patients from screening points to CXR/Radiology, clinical consultation, laboratory and other required services.

- Coordinate with facility staff to facilitate timely completion of advised CXR.





- Support patients in understanding the next steps in their care pathway.

- Help reduce delays and incomplete referrals within the facility.

- Diagnostic Facilitation and Follow-up

- Facilitate counselling and referral of presumptive TB patients for appropriate diagnostic evaluation.

- Support sputum/sample collection, laboratory registration and NAAT referral, as applicable.

- Track pending CXR, sample collection, diagnostic tests and results.

- Follow up with patients and relevant facility teams for completion of pending investigations.

- Facilitate clinical referral for patients requiring further evaluation.

- Support completion of the diagnostic pathway until a diagnostic decision is available.

- TB Treatment, NTEP and TPT Linkage

- Facilitate referral of diagnosed TB patients to appropriate facility/NTEP personnel.

- Support linkage with Ni-kshay notification, treatment initiation and NTEP services, in coordination with the responsible programme teams.

- Support referral of potential TPT-eligible individuals to NTEP for further eligibility assessment and management.

- Track pending referrals and linkages and coordinate follow-up as required.

- Data Collection and Project MIS

- Collect and record patient-level information using approved project formats and the Project MIS.

- Ensure timely, complete and accurate entry/update of screening, CXR, diagnostic, referral and follow-up information.

- Maintain required patient tracking and supporting records.

- Identify incomplete patient records and update them through appropriate follow-up.

- Work with District M&E; personnel to address data-quality issues and discrepancies.

- Maintain confidentiality and appropriate handling of patient information.

- Facility Coordination

- Coordinate routinely with OPD/IPD teams, clinicians, nurses, radiology staff, laboratory personnel and NTEP staff.

- Support smooth functioning of designated screening and patient-navigation points.

- Inform the District Coordinator about operational bottlenecks affecting screening or patient-care pathways.

- Participate in facility-level meetings and implementation reviews as required.

- Support implementation of corrective actions identified during monitoring and supportive supervision.

- Reporting and Project Support

- Provide routine updates on screening, patient navigation, pending investigations and follow-up activities.

- Participate in project orientation, training and refresher sessions.

- Support monitoring visits, data-quality reviews and supportive supervision.

- Maintain required activity and patient-level documentation.





Support other facility-level project activities assigned by the District Coordinator or project leadership.

Expected Deliverable

Field Officers will support routine patient enrolment and screening across assigned OPD/IPD points; timely and accurate patient-level data collection and Project MIS updates; facilitation of CXR and diagnostic pathways; tracking and follow-up of pending investigations; patient navigation through the care cascade; linkage with facility/NTEP services; and timely reporting of implementation progress and operational gaps.

Qualification

Minimum: Graduate degree/diploma in a relevant discipline such as Public Health, Medical and Allied Health Sciences, Nursing, Social Sciences, Health/Healthcare Management, or other health-related disciplines.

Desirable: Post Graduate degree in Public Health, Medical and Allied Health Sciences, Nursing, Social Sciences, Health/Healthcare Management, or other health-related disciplines.

Relevant training or experience in public health programmes, patient counselling/navigation, healthcare delivery, data collection or field-level programme implementation.

Experience

Minimum: At least 1 year of relevant experience in one or more areas such as public health, healthcare programmes, field-level implementation, patient management/navigation, data collection, community-based programmes, hospital/health-facility work or allied health services.

Experience requirements may be considered flexibly based on the candidate’s educational background and relevance of prior work.

Desirable: Previous exposure to government health programmes, healthcare facilities, TB/NTEP or other public health programmes will be an advantage but is not mandatory.
