Responsibilities

- Provide expert pharmaceutical care and advanced pharmacology consultation to patients and healthcare providers within the hospital setting.

- Develop and implement effective medication management plans for complex patient cases.

- Collaborate with multidisciplinary teams to optimize therapeutic outcomes and ensure secure medication practices.

- Educate patients and their families about medication therapy, potential side effects, and adherence strategies.

- Conduct medication reconciliation and review patient medication histories to prevent adverse drug interactions.

- Participate in the development and enforcement of hospital pharmacy policies and procedures.

- Support emergency care teams by providing timely pharmaceutical expertise in critical situations.

- Maintain accurate clinical documentation and ensure compliance with relevant regulations and standards.

- Stay current with the latest advancements in pharmacology and contribute to continuous improvement initiatives within the department.
