Role & responsibilities
Review and validate prescriptions for appropriate drug, dose, route, frequency, and duration.
Dispense medicines accurately to inpatients and outpatients.
Check medication orders for potential drug interactions, duplications, allergies, contraindications, and dosing issues.
Provide medication-related information and counseling to patients and healthcare professionals.
Maintain accurate records of dispensing, controlled medicines, narcotics, and other regulated medications as applicable.
Monitor pharmacy inventory, including stock levels, expiry dates, storage conditions, and batch details.
Ensure proper storage of medicines, including temperature-sensitive and high-alert medications.
Participate in medication reconciliation and support clinical pharmacy activities where applicable.
Coordinate with doctors, nurses, and other healthcare specialists regarding medication-related issues.
Report medication errors, adverse drug reactions, and other medication-safety concerns according to hospital procedures.
Follow hospital SOPs, infection-control requirements, and applicable regulatory standards.
Support pharmacy audits, accreditation activities, and quality-improvement initiatives.
Maintain confidentiality of patient and hospital information.
Perform other pharmacy-related duties assigned by the Pharmacy Manager.
