Medication Management
- Review and validate medication prescriptions for accuracy and appropriateness.
- Dispense medications according to physician orders and hospital protocols.
- Ensure proper storage, handling, and inventory management of pharmaceuticals.
- Monitor medication stock levels and coordinate procurement activities.
- Manage controlled substances in compliance with regulatory requirements.
Clinical Pharmacy Services
- Participate in patient care rounds and multidisciplinary healthcare teams.
- Review patient medication histories and identify potential drug interactions.
- Provide recommendations on dosage adjustments and alternative therapies.
- Monitor patient responses to medication and report adverse drug reactions.
- Support antimicrobial stewardship and medication safety initiatives.
Patient Counseling
- Educate patients and caregivers on medication usage, dosage, side effects, and precautions.
- Provide discharge medication counseling.
- Answer medication-related queries from patients and healthcare staff.
Compliance and Documentation
- Maintain accurate pharmacy records and documentation.
- Ensure compliance with hospital policies, pharmacy regulations, and accreditation standards.
- Participate in medication audits and quality improvement programs.
- Report medication errors and contribute to corrective actions.
Training and Development
- Train and supervise pharmacy technicians, interns,
and support staff.
- Participate in continuing education and professional development activities.
- Conduct medication awareness and safety programs for hospital staff.
Qualifications
- Bachelor's Degree in Pharmacy (B.Pharm) / Diploma in Pharmacy (D.Pharm)
- Registered Pharmacist with the relevant Pharmacy Council/Regulatory Authority.
- Minimum 1-3 years of hospital pharmacy experience (preferred).
