We are looking for a qualified and experienced Clinical Pharmacist to join our hospital team. The candidate will be responsible for ensuring secure and effective medication use and supporting doctors and other healthcare professionals in patient care.

Key Responsibilities

- Review and validate medication orders and prescriptions.

- Monitor patients’ medication therapy and identify potential drug interactions.

- Provide appropriate recommendations to doctors regarding medication selection, dosage, and administration.

- Monitor adverse drug reactions and medication-related problems.

- Provide medication counseling and education to patients and caregivers.

- Maintain accurate medication and clinical records.

- Participate in clinical rounds and multidisciplinary healthcare team discussions.

- Ensure compliance with hospital pharmacy policies and applicable regulations.

- Support medication safety and rational drug-use initiatives.

Qualifications

- Pharm.D / M.Pharm in Pharmacy or a relevant qualification.

- Valid registration with the State Pharmacy Council.

- Previous hospital/clinical pharmacy experience preferred.

- Good communication and interpersonal skills.
