About the Role
The Care Coordination Specialist ensures the smooth delivery of care services by managing staff schedules and monitoring care calls. The role involves effective communication with clients and healthcare professionals, addressing concerns, and overseeing staff performance through analysis and monitoring.
Responsibilities
• Creating staff rotas and managing schedules
• Handling inbound and outbound Staff/Client calls
• Monitoring and covering care visits
• Following our processes, policies and care service standards
• Communicating with field staff, clients and healthcare professionals




• Addressing staff and client concerns, and providing timely, accurate and effective solutions
• Carrying out staff performance analysis and mentoring
• Managing care plans and reviews
• Gathering feedback and implementing service improvements
• Completing any additional tasks assigned by management

Qualifications
• Any Graduate/Diploma
• Previous work experience in customer service/support, operations coordination, recruitment, or client support roles. (Highly Preferred)

Must Have
• Experience supporting customers/clients by both phone and email.
• Fluent in written and spoken English

Comfortable working in Night Shift/Rotational Shift & weekends
