Before sending a claim, check CPT, HCPCS, ICD/-10/-CM codes, and modifiers against medical records, NCCI revisions, and payor coverage criteria to remove coding errors. To avoid registration/-related claim denials, cross/-check patient demographics, insurance coverage information, policy numbers, rendering/billing provider NPIs, and pre/-authorizations. To maintain high Clean Claim Rates, review claim scrubber rules, fix front/-end claim modifications, fix missing information, and flag missing charge documentation.
