• Coordinating patient care throughout the transplant process, including evaluation, admission, and post-operative care
• Educating patients and their families about the transplant process, including potential risks and perks
• Serving as a liaison between the transplant team, patients, and their families
• Collaborating with healthcare professionals to ensure comprehensive care for transplant patients
• Maintaining detailed records of patient interactions and progress
• Participating in quality improvement initiatives to enhance patient outcomes
• Adhering to all regulatory requirements and ethical standards in healthcare
• Bachelor's degree in Nursing or related field required
• Valid state nursing license and CPR certification
• Strong communication and interpersonal skills
• Ability to work both independently and as part of a multidisciplinary team
• Exceptional organizational skills and attention to detail
• Previous experience in transplant coordination preferred, but not required
• Knowledge of transplant process and healthcare regulations
