Role & responsibilities
Review and validate prescriptions to ensure protected and appropriate medication use.
Monitor patients' medication therapy and recommend necessary adjustments.
Identify and prevent drug interactions, adverse drug reactions, and medication errors.
Provide medication-related information and support to doctors, nurses, and other healthcare specialists.
Counsel patients on medication usage, dosage, side effects, and adherence.
Participate in clinical rounds and contribute to patient care decisions.
Monitor and document adverse drug reactions (ADRs) and medication incidents.
Ensure compliance with hospital policies, regulatory requirements, and medication safety standards.
Maintain accurate clinical pharmacy records and reports.
Conduct medication audits, drug utilization reviews, and educational programs for healthcare staff.

Preferred candidate profile
Female candidate are preferred.
Qualification - Pharm.D Pharm.B
Candidates having 2 years experience in oncology are preferred
