- BAMS (Bachelor of Ayurvedic Medicine and Surgery) from a recognized institution/university.
- Valid registration with the respective State/Indian system of medicine council, as applicable.
Experience
- Minimum 1 year of relevant clinical experience required.
- Experience in community healthcare, primary healthcare, rural healthcare will be preferred.
Key Responsibilities
- Review and verify patient medical records for completeness, accuracy, and proper documentation.
- Ensure clinical records, diagnosis, treatment details, discharge summaries, and other documents are complete and properly authenticated.
- Conduct medical record audits and identify documentation gaps or discrepancies.
- Coordinate with doctors, nursing, and clinical departments for completion and correction of medical records.
- Assist in maintaining medical records as per hospital policies and NABH/NMC requirements.
- Support preparation of MRD statistics, MIS reports, and clinical documentation-related reports.
- Ensure proper filing, indexing, retrieval, and confidentiality of patient medical records.
- Assist during internal, NABH, NMC, and other regulatory audits.
- Support medico-legal documentation and ensure records are maintained as per applicable guidelines.
- Coordinate with Billing, TPA/Insurance, Quality, and other departments for medical record-related requirements.
- Perform other duties related to MRD as assigned by the reporting manager.
