Clinical Pharmacist The Clinical Pharmacist is responsible for providing patient-focused pharmaceutical care, ensuring the safe, effective, and appropriate use of medications. The role involves reviewing prescriptions, monitoring medication therapy, identifying drug-related problems, supporting physicians and clinical teams, and providing medication-related counselling to patients.
Qualification:
- B.Pharm / M.Pharm (Pharmacy) from a recognized institution.
- Pharm.D preferred for clinical pharmacist roles.
- Valid registration with the State Pharmacy Council / Pharmacy Council of India, as applicable.
- Hospital/clinical pharmacy experience preferred.
Roles & Responsibilities:
- Review and evaluate prescriptions and medication orders for accuracy and appropriateness.
- Monitor patients for drug interactions, adverse drug reactions, contraindications, and medication errors.
- Provide appropriate medication-related recommendations to physicians and clinical teams.
- Participate in ward rounds and multidisciplinary clinical discussions where applicable.
- Monitor medication therapy and patient response to treatment.
- Provide drug information to doctors, nurses, and other healthcare professionals.
- Counsel patients regarding medication dosage, administration, precautions, side effects,
and adherence.
- Assist in medication reconciliation during admission, transfer, and discharge.
- Monitor high-risk medications and support protected medication-use practices.
- Maintain proper documentation of clinical pharmacy activities and interventions.
- Support antimicrobial stewardship and rational antibiotic use.
- Assist in developing and maintaining medication-related protocols, guidelines, and formulary information.
- Report and document Adverse Drug Reactions (ADRs) and medication-related incidents.
- Coordinate with the pharmacy, nursing, medical, and other clinical departments.
- Ensure compliance with hospital policies, medication-safety standards, and applicable regulatory requirements.
- Participate in quality improvement, clinical audits, training, and pharmacy-related hospital activities.
